Group Term Life Insurance

Accidental Death & Dismemberment (AD&D) Insurance
- Company-paid protection covering accidental death or dismemberment
- 2x annual base salary coverage; $1 million maximum
Short-Term Disability (STD)
- Non-Occupational: combination of full pay and half pay, up to 26 weeks’ full pay, based on years of service with the company
- Occupational: up to 26 weeks’ full pay, then up to 26 weeks half pay
- 7-day waiting period per disability before benefits begin
- Cost is paid by the company
Long-Term Disability (LTD) Insurance
- After 26 weeks of disability, provides 60% of pre-disability monthly earnings up to a $10,000 monthly benefit
- Benefits are taxable under IRS regulations

Voluntary Group Term Life Insurance
- Portable financial protection for you and/or your survivors to help meet
financial obligations in the event of death:
- 1x to 6x salary, up to $1 million
